1. About this policy
Health Visit App, operated by Miami App Studio, complies with the U.S. Digital Millennium Copyright Act ("DMCA"), 17 U.S.C. § 512. This page explains how copyright owners (or their agents) can submit a takedown notice, and how users whose content has been removed can submit a counter-notice.
We reserve the right to terminate the accounts of users we determine to be repeat infringers.
2. How to submit a DMCA takedown notice
If you are a copyright owner (or authorized to act on behalf of one) and you believe that material accessible via Health Visit App infringes your copyright, send a written notice to our Designated Agent (below) that includes all six of the following:
- A physical or electronic signature of the copyright owner or a person authorized to act on their behalf.
- Identification of the copyrighted work claimed to have been infringed (or a representative list, if multiple works).
- Identification of the material that is claimed to be infringing, with enough detail (URL, screenshot, account name) for us to locate it.
- Your contact information: full name, mailing address, telephone number, and email address.
- A statement that you have a good-faith belief that the use of the material is not authorized by the copyright owner, its agent, or the law.
- A statement, made under penalty of perjury, that the information in the notice is accurate and that you are the copyright owner or are authorized to act on the owner's behalf.
Incomplete notices may not be acted on. Filing a false notice can subject you to liability for damages, including attorneys' fees.

Email is the fastest way to reach us. Please include "DMCA Takedown Notice" in the subject line so we can route it correctly.
4. What happens after we receive a notice
- We review the notice for completeness and good faith.
- If valid, we remove or disable access to the allegedly infringing material expeditiously.
- We notify the user who posted the material and forward your notice to them (your contact information may be shared).
- We log the notice and may apply it toward our repeat-infringer policy if the user receives multiple verified notices.
5. How to submit a counter-notice
If your content was removed and you believe it was a mistake or misidentification, you may submit a counter-notice. The counter-notice must include all of the following:
- Your physical or electronic signature.
- Identification of the material that was removed and the location where it appeared before removal.
- A statement, under penalty of perjury, that you have a good-faith belief the material was removed as a result of mistake or misidentification.
- Your full name, address, telephone number, and email.
- A statement that you consent to the jurisdiction of the U.S. Federal District Court for the district in which your address is located (or, if outside the U.S., the Southern District of Florida), and that you will accept service of process from the person who provided the original notice or their agent.

6. What happens after a counter-notice
If we receive a valid counter-notice, we will forward it to the original complainant. If they do not file a court action seeking an order against you within 10 to 14 business days, we may restore the removed material.
Filing a false counter-notice can subject you to liability, including damages and attorneys' fees.
7. Repeat infringer policy
In appropriate circumstances, we will terminate the accounts of users who are determined to be repeat infringers. We may also disable access to material that is the subject of repeated valid notices.
8. Important note for HealthVisitApp users
Health Visit App is a private, account-based service used by individuals and family caregivers to organize their own personal health information. The app is not a public publishing platform, and content uploaded by users is generally visible only to themselves and (on Family plans) to people they explicitly share with. As a result, copyright takedown issues are rare. This policy exists so that, in the unusual case where one is needed, the process is clear.
